It's a difficult question. I think it might be a Borrelia infection. It's louse-bourne, so scratching explains excoriations. The disease is found in Africa. Simptoms: rigors, headache, delirium, myalgia, jaundice, a petechial or ecchimotic rash. Tetracycline or erythromycin are used against Spirochaets. Spirochaets are found microscopically in the blood.
