I think this is an extremely imp question for PLAB. Jehovas witness is a curious sect with a staunch belief that if they accept blood transfusion, their soul becomes impure and therefore their religion very strictly forbids them from having any form of blood or blood product transfusion. It is a very common situation in this country n all these people carry an advance legal directive which atates that no doctor or health professional can administer a blood transfusion under any circumstance. Surgical importance is that even mild anaemia in these patients will have to be corrected by use of iron or recombinant erythropoeitin in elective surgery.In vasculer n cardiac surgery in this group of patients, meticulous hemostasis is the rule and cell-saver machines which recycle the patients shed blood and allow retransfusion are used. This is the only method of transfusion that is acceptable to some of these people.
