I also thought the same ans because there are signs of shock present & the clue here is avoiding episiotomy. If epi is not given there is chance of vag tear & ut rapture due to forcefull pushing during labour,in vag tear the bleeding is unlikely to cause shock. So the ans is ut rapture to this quest.
