c) laparoscopic mediastinotomy

1) A patient is suspected of having bronchiectasis
2) a patient with an area of consolidation and fever on his chest xray is treated with antibiotics. His fever resolves but four weeks later he still has a large area of consolidation
3) A singer has a six month history of hoarseness
4) A 55 year old smoker complains of shortness of breath and a cough. Laryngoscopy is normal
5) A 30 year old complains of hoarseness since he came back from a football match the day before
